Preadmission Screening Specialist

Galt Foundation is seeking a Preadmission Screening Specialist to perform functional needs assessments for persons with developmental disabilities to determine the person's level of support needed.

Job Duties

  • Utilize tools to assess needs, including but not limited to: Oregon Needs Assessment (ONA), Foster Care Support Needs Assessment, and Supports Intensity Scale (SIS)
  • Coordinate scheduling and interview clients, clients' families, service providers, etc. to evaluate psychological, cultural, financial, and medical conditions of people with developmental disabilities.
  • Provide in-depth consultation to community professionals, agency staff, providers, or other persons seeking guidance in meeting client needs.
  • May provide crisis assessment using the ONA, SIS, SNAP, or other assessment measures and participate in design of appropriate placement and interventions as indicated in the client assessment.
  • Maintains up to date knowledge of State and Federal rules and regulations regarding services delivery and special technical knowledge regarding medical, psychological, behavioral issues, and appropriate interventions.

Job Requirements

  • Must meet one of the following criteria:

    • (a) A bachelor's degree in behavioral science, social science, or a closely related field

    • (b) A bachelor s degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing;

    • (c) An associate s degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or

    • (d) Three years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing.

  • Must participate with a criminal background check and drug screening.
  • Must be 18 or older

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
