Mental Health Specialist I - WRAP Care Coordinator


Job Description
POSITION SUMMARY:
The Wrap Care Coordinator (WCC) is responsible for a wide range of activities which are expected to evolve over time, depending on program need and state requirements.
ESSENTIONAL JOB DUTIES:
  • Coordinating mental health services with outside agencies, including placement agencies, Developmental Disability services, and educational services;
  • Assisting referrers with the collection of information for referral packets and serving as the point of contact for submission of information;
  • Providing training, technical assistance, and/or instruction on the Intensive Service Array (ISA) Program;
  • Facilitating communication between the family, treatment providers, natural supports, community resources, and involved child-serving providers and agencies;
  • Providing orientation to ISA services for children and families;
  • Organizing and facilitating Community Resource Team (CRT) meetings that identify the strengths and needs of children and families referred for ISA services and develop service coordination plans and safety and crisis plans;
  • Providing for continuity of care by creating linkages to and managing transitions between levels of care and transitions for older youth to the adult service system;
  • Assisting with coordination and tracking of children in inpatient care;
  • Coordinating payments for mental health services with other counties and local service providers;
  • Coordinate and participate in reviews of all ISA cases every 90 days, and individual reviews for Day Treatment clients every 90 days;
  • Collection and submission of data to: GOBHI, Regional Youth Resource Program, and OMHAS for all ISA children;
  • Provide direct services to children and families..

Job Requirements
COMPENTENCIES AND SKILLS REQUIREMENTS:
Must be able to travel to various sites within the county, and occasionally within the state.
EDUCATION AND EXPERIENCE :
  • A bachelor's degree or equivalent course work in a field related to management such as business orA Bachelor s degree in a Social Science field and at least 2 years of experience working with children and families with preferred experience in WRAP services or Multi-System Therapy.
  • Master s degree preferred.

PHYSICAL REQUIREMENTS
The physical demands described here are representative of those that must be met by an employee to successfully perform the job. The employee is frequently required to sit, walk, or stand; use hands and fingers to handle or feel; reach with hands and arms; and ability to talk and hear. The employee must regularly lift and/or move up to 20 pounds. Specific vision abilities required by this job include close, distance, color, and peripheral vision, along with depth perception and ability to adjust focus. Regular over-night and out of office travel is required.
  • Hazardous Conditions : Risk of verbal and physical threats from clients. Use of keyboard and computer monitor (repetitive hand motions and potential eye strain). Possible exposure to airborne pathogens.
  • Working Conditions : The noise level in the work environment is mild to moderate; 20% office-based, 80% field.
  • Equipment / Materials Used : Common office machines including computer, printer, calculator, telephone equipment, tele-health system, copier, scanner, and facsimile. May drive your personal or Agency vehicle.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
